CaringHand™ Medical Group ("CaringHand Medical", "we", "our" or "us") provides medical and health services through licensed physicians and healthcare professionals (the "Providers") specializing in cancer survivorship care, with support from our technology partner, Adaptic Health, Inc. ("Adaptic Health"). This Informed Consent describes CaringHand's services and clinical programs. It is important for you to read this document and discuss any questions you might have with us.
By using the Services and creating an account, you acknowledge that you have read, understood, and agree to this Informed Consent. Where applicable, you further certify that you have legal authority to act as guardian or personal representative of all individuals registered under your account.
Our Services and Technology
Comprehensive Survivorship Support
When you become a patient of CaringHand Medical (a "Member"), you will be given access to:
- CaringHand Platform: Our website and mobile applications that serve as your hub for cancer survivorship resources and care coordination
- CaringHand AI Assistant: An AI-powered assistant that provides educational information and support 24/7 to help our clinical team serve you better
- Healthcare Provider Team: Licensed clinicians specializing in cancer survivorship who provide personalized medical care supported by AI
- Educational Resources: Personalized content, interactive tools, and evidence-based information about cancer survivorship
About CaringHand AI
CaringHand AI is an AI-powered assistant powered by advanced language models that allows you to ask questions and receive real-time answers and educational content about cancer survivorship, supporting our clinical team in delivering comprehensive care.
Important Limitations: You understand that while CaringHand AI may provide informational content and educational support, it does not provide medical or clinical advice, cannot make diagnoses, and is not a licensed clinical provider. Telehealth Services - Risks and Benefits
Nature of Telehealth
Our Providers will interact with you via telehealth, primarily using asynchronous messaging and video consultations when appropriate. They may prescribe medication, order laboratory tests, or recommend other treatment as needed. Telehealth is a flexible and convenient way to receive healthcare, but it may not be right for treating certain symptoms or illnesses that need an in-person doctor visit or urgent care.

Risks and Limitations of Telehealth
As with any health service, there are potential risks associated with using telehealth technology:
- Technology Issues: Service problems due to technology failures, internet connectivity issues, or equipment malfunction
- Limited Physical Examination: Providers cannot perform hands-on physical examinations, which may limit diagnostic capabilities
- Information Limitations: Providers may not have all the information needed to make certain healthcare decisions without in-person evaluation
- Security Risks: While we use secure, HIPAA-compliant technology, there is always a rare risk of security breaches
- Technical Quality: Audio or video quality may vary and could affect the consultation
You accept these inherent risks of telehealth and acknowledge the risk of information loss due to technology problems.
Primary Care Coordination
Responsibility for your overall medical care should remain with your primary care provider. CaringHand™ Medical specializes in cancer survivorship care and is designed to complement, not replace, your relationship with your primary care physician and oncology team.
Emergency Care Warning
If you are having a medical emergency, call 9-1-1 or go to the nearest emergency room.
CaringHand™ is not an emergency service and should not be used for urgent or emergency medical needs. Examples of emergencies include:
- Chest pain or difficulty breathing
- Severe bleeding or injuries
- Loss of consciousness
- Severe allergic reactions
- Thoughts of hurting yourself or others
- Any other life-threatening condition
State Licensing and Service Availability
Our Providers are licensed to practice medicine in specific states. We can only provide services to patients who are physically located in states where our Providers hold active medical licenses at the time of the consultation.
You agree to provide accurate information about your physical location when using our Services. Receiving services when located in a state where our provider is not licensed may be illegal and could result in termination of your account.
Privacy and Security
We are committed to protecting your privacy and health information:
- All communications are conducted through HIPAA-compliant, encrypted platforms
- Your health information is protected according to federal and state privacy laws

- We use industry-standard security measures to protect your data
However, you acknowledge that no method of electronic transmission or storage is 100% secure, and we cannot guarantee absolute security of your information.
Patient Responsibilities
To ensure the best care and outcomes, you agree to:
Provide Accurate Information
- Provide complete and accurate medical history, including all medications and supplements
- Disclose all relevant health conditions and symptoms
- Update your information if your health status changes
- Provide accurate location information for each consultation
Follow Treatment Plans
- Follow prescribed treatment plans and medication instructions
- Attend scheduled follow-up appointments
- Communicate with your providers about any concerns or side effects
- Inform your providers if you are unable or unwilling to follow recommendations
Use Services Appropriately
- Use emergency services (911) for medical emergencies
- Maintain regular care with your primary care physician
- Ensure adequate internet connectivity and technology for telehealth visits
- Conduct consultations in a private, quiet location when possible
- Not share your account credentials with others
Provider Responsibilities
Our Providers commit to:
- Provide care within their scope of practice and licensure
- Exercise professional judgment in determining appropriate treatment
- Maintain confidentiality and privacy of your health information
- Communicate clearly about treatment plans and recommendations
- Refer you to in-person care when telehealth is not appropriate
- Coordinate with your other healthcare providers when appropriate
Prescriptions and Medications
Our Providers may prescribe medications when medically appropriate. You understand that:
- Prescriptions will be sent to your preferred pharmacy electronically
- Certain controlled substances may not be prescribed via telehealth
- You are responsible for picking up and taking medications as prescribed
- You must inform your provider of all medications you are currently taking
- Some medications may not be covered by your insurance
- State and federal laws govern what can be prescribed via telehealth
Laboratory Testing
When medically necessary, our Providers may order laboratory tests. You understand that:
- You will need to visit a laboratory facility in person for sample collection
- Additional costs may apply for laboratory services
- Results will be communicated to you through the CaringHand platform
- Follow-up care will be coordinated based on test results
Payment and Billing
Subscription Fees
CaringHand™ operates on a subscription model. Each bill for all charges must be paid by the date shown on the billing statement. If you choose to pay your bill using a credit card, you authorize us to charge the provided credit card number for the agreed-upon services and use your credit card details for future transactions on your account.
HSA/FSA Eligibility
Our medical services are generally eligible for payment through Health Savings Accounts (HSA) or Flexible Spending Accounts (FSA). Please consult with your plan administrator to confirm eligibility and required documentation.
Technical Issues
Your charges will not be refunded in the event that you cannot complete a session due to an interruption in your internet service or other malfunction of your technology, unless the technical issue was caused by our platform.
Cancellation and Refund Policy
You may cancel your subscription at any time through your account settings. Cancellations take effect at the end of your current billing period. We do not provide refunds for partial subscription periods, except as required by law or in cases of extraordinary circumstances at our discretion.
Medical Records
You have the right to access your medical records as described in our HIPAA Notice of Privacy Practices. Electronic copies of your medical records are available through your account. You may also request paper copies by contacting us.
We recommend maintaining your own records of all communications, treatment plans, and medical information for your personal health management.

By using CaringHand's Services, you acknowledge that you have:

- Had the opportunity to ask questions about our services
- Understand the benefits and risks of telehealth services
- Understand the limitations of AI technology like CaringHand AI
- Agree to follow the patient responsibilities outlined above
- Consent to receive medical care via telehealth
- Understand that you can withdraw consent at any time by discontinuing use of the Services
